Occurrence Forms

HFSS uses incident reporting to consistently track, document, and respond to situations that impact swimmer safety, lesson flow, and staff accountability. These reports help leadership identify patterns, address risks, and ensure appropriate follow-up.

Site Supervisors are responsible for completing the required form anytime a qualifying incident occurs and for notifying leadership within the same day. Emergency situations should always follow 911 protocols first.

To track how often fecal incidents occur and their impact, Site Supervisors must complete this form anytime lessons are interrupted due to a fecal incident.

Report of Injury or Illness (ROI)

Report of Injury or Illness - Employee  Report of Injury or Illness - Guest

Use the appropriate Report of Injury or Illness form (Guest or Employee) anytime an injury or illness occurs that does not require a 911 response.

  • The Site Supervisor on shift completes the form in full
  • Submit the report by scanning and emailing it to the GM immediately
  • The GM reviews and forwards the report to Judy and Pat the same day
  • If the situation feels urgent or unclear, call your GM

These forms are for documentation and follow-up, not emergency response.

 
 

Unseen Submersion Report

Unseen Submersion Report

An unseen submersion occurs when a swimmer goes under water during a lesson without staff seeing the entry.

This form must be completed by the Site Supervisor on shift. The same day, scan and email the report to the GM. If needed, call your GM.

This form is not for emergency (911) situations.
