The HFSS Q12 is a supportive tool for employee engagement. We look at the Q12 as a method to say “how are YOU doing?” or “what’s it like to be a part of the HFSS Team?”
HFSS Q12 is based on Gallup’s Q12 Employee Engagement Survey. To fit the HFSS environment, we made some minor modifications while maintaining the scientific integrity.

Main objective is simple - to create conversation between employees and managers. Q12 is not anonymous because it is designed to collect information specific to individuals. It opens the opportunity to get feedback and then to follow up with more conversation. It provides talking points for the conversation between a supervisor and employee. It is a conversation that is driven by being “curious not critical.” The conversation which includes, but not limited to, “what’s going well” and “how can we be better?” should on average take about 15 minutes or so.
Examples of some possible thoughts to resolve:
- I see you gave a score of 2, what would need to be done to make it a 3 or higher.
- Great to see the 5s. Tell me why you scored it as a 5.
- You scored a 2 on ___, is it always a 2 or does it depend on the day.
- Dig into questions to find other people's names or patterns.
- Be curious and ask questions to verify the validity.
What to look for…
- Mistakes in entering wrong score or misinterpreting the questions
- Kernels of truth - the answers are all the truth because it is how they feel. What is the underlying reason?
- Patterns. Are there positive or negative feelings towards similar people?
- How can HFSS be better?

Timeline on the Operations Calendar (dates below are estimates)
- Oct 1 - Review questions and intake spreadsheet
- Oct 15 - Inform through Ops L-10 and SS L-10
- Week before distribution - note in MM with completion due date and “if you didn't get it”
- Nov 1 - Distribute via text to all HFSS
- Review results
- Delegate 1:1 meetings, print individual results for meetings
